community health worker (CHW) is a frontline public health worker and a trusted member of the community. Community health workers boost health knowledge, bridge gaps between clinical health care providers and their patients/clients, and help people navigate complex systems. They have an in-depth understanding of a community’s needs, often based on their lived experience. This strong bond allows CHWs to act as a link between health and social services and the people they serve.

Connecticut defined CHWs in statute in 2017 and created a voluntary CHW certification process in 2019. There are currently 234 certified CHWs in CT.
